4. Name and quantity of material(s) included to the extent known.

5. The extent of injuries, if any.

6. Possible hazards to human health or the environment, outside the facility

1. Immediately or within 24 hours.

2. Agencies may require a follow-up report in writing.

3. Consult each agency for their reporting requirements.
