Include a procedure to investigate occupational injury or occupational illness.

 

PROCEDURE I.E.

Investigation of Injury and Illness

 

All injuries to students and employees will be investigated as soon as possible, and no later than 24 hours after the injury is reported. this shall be the responsibility of the immediate supervisor and the Risk Manager.

 

Investigations should focus on the following:

  1. What happened.

  2. Why it happened.

  3. Identification of any unsafe act or condition or both.

  4. Steps taken to correct the act and/or condition.

Corrections of unsafe acts and/or conditions should be made as soon as possible after they have been identified.

 

Site Safety Inspection Teams should review all injuries at the site to look for trends, make sure correction has been accomplished, and ensure that other employees doing the same work are aware of the hazard and/or correct method to avoid injury.

 

The District Safety Committee shall also meet and discuss all student and employee injuries that have occurred since the last meeting and recommend corrective action, review reports of unsafe condition, and consider the need for employee training in safety and health matters.

 

A written report on safety and health matters shall be prepared by the District Safety Officer and forwarded to the Chancellor at least once annually.

 

The District Safety Officer shall record which sites have complied with this policy, and shall review the reports and follow up to ensure that recommendations made by Teams have been addressed.
