INJURY AND ILLNESS PREVENTION PLAN

1. The program must be in writing:

   1. One or more people are identified as having both authority and responsibility for implementing the Program.

   2. A system(s) is (are) in place to ensure employee compliance with safe and healthy work practices.

   3. A system is established for communicating with employees on safety and health matters.

   4. A system is in place to identify and evaluate work place hazards.

   5. Investigations must be made of injuries and illnesses.

   6. A method and procedure has been established for correcting unsafe conditions and practices.

   7. A training program is in place.
