Records of the steps taken to implement
and maintain the Program shall include:
Records of scheduled and periodic inspections required by
subsection I.D. to identify unsafe conditions and work practices,
including person(s) conducting the inspection, the unsafe conditions
and work practices that have been identified, and action taken to
correct the identified unsafe conditions and work practices. The
records shall be maintained for three (3) years; and
Documentation of safety and health training required by
subsection I.G. for each employee, including employee name or other
identifier, training dates, type(s) of training, and training
providers. This documentation shall be maintained for three (3) years.
Exception: Training records of
employees who have worked for less than one (1) year for the employer
need not be retained beyond the term of employment if they are
provided to the employee upon termination of employment.
PROCEDURE I.I.
Record Keeping
The Vice Chancellor Business, shall be the
District Safety Officer and will be responsible for assuring the
maintenance of all records required and necessary for documenting how
the district's injury prevention program is being implemented.
A copy will be maintained by the District
Safety Officer or designee. All required records must be maintained
for three (3) years.
The following records, at a minimum, will
be maintained:
Training records for each employee, including the employee's name,
date of training, subject of training, and the training provider are
to be kept.
Records of inspections must be maintained and must identify the
person(s) who conducted the inspection, the unsafe conditions and/or
work practices that have been identified, and any action taken to
correct the unsafe condition and/or work practice.
Records of any disciplinary action taken involving employee
violations(s) or safety and/or health rules will also be maintained.
